MINE ID # OR PROSPECTING ID#: M-1983-141 PAGE: 3 <br />INSPECTION DATE: 5-6-09 INSPECTOR'S INITIALS: ACS <br />a short distance west of the intersection with the haul road from the <br />Cash and Mount Royale areas to the mill. Based on observations made <br />during this inspection, the following questions need to be answered <br />(see PB-3 on page four of this report). <br />1. What are the permit nos. assigned by the Division of Water Resources <br />(State Engineer) for the wells at location CM-4 as labeled on the AM-02 <br />map? Does the State Engineer require disinfection of these wells when <br />the seals are removed? <br />2. What are the water rights implications related to pumping the Cash <br />Mine well and dewatering of the mine in general? What authorizations <br />have been obtained from the State Engineer to store and use Cash Mine <br />water at the Gold Hill Mill? <br />3. Describe the locations, purposes, and uses of each of the pipes, <br />tubes, hoses and cables strung throughout the site. <br />4. Has pumped Cash Mine water ever been discharged to the Who Do Shaft? <br />Provide a list of locations where Cash Mine water has been discharged <br />for storage, use, or disposal. Is Cash Mine dewatering necessary to <br />prevent the mine pool from spilling into and discharging from the Mount <br />Royale Adit? <br />5. Where is the inlet and what is the purpose of the pipe discharging <br />onto the Who Do Mine road as discussed above?
