(Page 4) <br />MINE ID # OR PROSPECTING ID #: M-1983-067 <br />INSPECTION DATE: May 272009 INSPECTOR'S INITIALS: DR? <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION DATE <br />1. Problem: DRMS believes that the slurry wall has created a ground water dam and is causing <br />elevated water levels in the Zigan Lake and in ground water upgradient of the mine site. If allowed <br />to proceed unabated, this may constitute a violation of §34-32.5-116(4)(h) of the Construction <br />Materials Act for failing to minimize disturbance to the prevailing hydrologic balance. <br />Corrective Action: Submit a Technical Revision to implement mitigation that will provide short-term- <br />relief. and an Amendment to implement mitigation that will provide long-term relief, to the <br />mounding that is occurring in the water table upgradient of the slurry wall.
