ARCADIS <br />4. Monitoring Well Installation and Abandonment Activities <br />In April 2009, Holcim retained ARCADIS' services to install one new groundwater <br />monitoring well downgradient of the quarry operations, and to abandon three <br />groundwater monitoring wells that were no longer active. Details of the field activities <br />performed as part of the groundwater monitoring well installation and abandonments <br />are described in Sections 4.1 through 4.4. <br />4.1 Pre-Field Activities <br />Pre-field activities included performing a utility clearance prior to the well installation <br />activities. Historical maps of the site were inspected to identify any potential historical <br />utilities in the vicinity of the proposed well location. The Colorado One-Call service was <br />contacted and notified of the planned well installation activities at least 48 hours before <br />initiating intrusive activities, and representatives from Colorado Interstate Gas and <br />Qwest Communications Network went to the site to inspect and identify any public <br />utility lines. The representative from Colorado Interstate Gas identified and marked out <br />an abandoned gas line in the vicinity of the proposed well location. Therefore, the <br />location of the well was moved slightly to the northwest to be at least 10 feet away from <br />this abandoned gas line. <br />A Notice of Intent was submitted to the Colorado Division of Water Resources (DWR) <br />on March 20, 2009 to notify the DWR of Holcim's intent to install a new groundwater <br />well for monitoring purposes. A copy of the Notice of Intent is included in Appendix E. <br />Site-specific training was performed prior to any field activities. In addition, the field <br />crews reviewed ARCADIS' Health and Safety Plan (HASP) and applicable drilling and <br />well installation Job Safety Analyses (JSAs) prior to starting field work (ARCADIS <br />2008). <br />4.2 Monitoring Well Installation Activities <br />Drilling was initiated on April 2, 2009 to install the new groundwater monitoring well <br />(MW-13) using an ODEX drilling rig with air rotary capabilities. Five-inch drill pipe with a <br />carbide bit was used to advance the borehole. Soil samples were collected from the air <br />stream approximately every few feet for lithologic logging.
