This operation, and the corresponding active permit, <br />were put on temporary cessation status on 12/03/95, and remain in temporary cessation to date, with <br />only environmental monitoring and maintenance occurring on-site. Attached is the completed <br />application form for "Renewal of a Valid, Existing Permit to Conduct Mining in Colorado". The <br />permit should be renewed for an additional term of five years in the name of the current permittee, as <br />follows: <br />BTU Empire Corporation <br />29515 RCR27 <br />Oak Creek, Colorado 80467 <br />(970) 879-3800 <br />Attached, consistent with the applicable requirements for permit renewal of Rule 2.08.5(2)(b), are the <br />following supplemental materials: <br />Copy of proposed Public Notice, consistent with applicable requirements of Rule 2.07.3(2) <br />Copy of current liability insurance policy (renewed annually each year during the term of the <br />active mining permit) <br />We appreciate your consideration, cooperation, and assistance in facilitating renewal of our Mining <br />and Reclamation Permit, and look forward to working closely with you to coordinate on review and <br />resolution of any renewal issues.
