(Page 2) <br />MINE ID # OR PROSPECTING ID #: 44-1980-244 <br />INSPECTION DATE: 711409 <br />INSPECTOR=S INITIALS: _RK <br />OBSERVATIONS <br />The Division conducted a monthly monitoring, construction progress of the Amendment 9 areas and a third quarter water <br />sampling from the under drain and the three low volume solution collection discharges for was cyanide analysis. Present <br />during the construction portion of the inspection was Mr. Tim Comer, the new environmental manager for the site. <br />Erath moving operation of the areas approved under Amendment # 9 is continuing. The New LOB foundation is in place. <br />Also the new haul road leading to the LOB is also completed. Removal of the old Rubi Leach pad in anticipation of the <br />mining extension of the main Cresson mine is continuing and should be completed by the next inspection date next <br />month. <br />The reclaimed slope for the noise berm along God Field south of the East Cresson mine appear to be holding well. No <br />major sign of erosion features were noted given the slope being at angle of repose. Once the mining is completed in the <br />are the final slope for these berms if left in place and not utilized to backfill any of the pit will be an average slope of 2.5:1 <br />Some sign of vegetation cover was noted at the time of the inspection <br />After the construction progress inspection, met the new general manger for the site Mr. Ben Gunther and briefly talked <br />about the site . Mr. Peter O'Conner, of Anglo Gold was also present at the brief introduction with the new General manger <br />for the site. <br />After the brief meeting, with Mark Vanoni present inspected the existing valley leach facility . Storm water maintenance was <br />taking place along the access roads to the valley leach facility at the time of the inspection. Additional sumps as need have <br />been constructed as need to meet their requirements of the storm water permit for the site. <br />Collected the following readings from the High and Low Volume solution collection ponds for Phase I,II, III & IV and an 8 <br />ounce sample from each of the three low volume outlets for the ponds was collected for wad cyanide analysis. <br />Phase I High volume solution collection transducer # 1 was recorded at 29.3 # 2 was at 38.5 and # 3 was at 33.7. Low <br />volume solution collection transducer # 1 was at.35 and # 2 was at.34. Piezometer for the pond was recorded at 52.4 <br />feet. CC&V has agreed until the calibration of the pond is verified, DRMS will use the highest reading for the pond <br />at any given time as the level for the pond and the 80% operational permit condition. <br />Phase II & III High Volume solution collection transducer # 4 was recorded at 21.4 # 5 was at 16.8 and #6 was at 20.1. Low <br />volume solution collection transducer # 1 was at.20 and # 2 was at .48 Piezometer for the pod was at 28.0 feet. <br />Phase IV High volume solution collection transducer # 7 was recorded at 22.3 # 8 was at 23.4 and # 9 was at 22.7, the pipe <br />or # 10 was at 22.6 Low volume solution collection transducer # 1 was at 15.0 and # 2 was at 9.1 <br />Inspected the external pond, it was holding some water. The low volume solution collection transducer # 1 was recorded at <br />11.2 and # 2 was at 9.8 inches of hydrostatic head. <br />Inspected the under drain both the south and 4 inch pipes were discharging and an eight ounce sample for wad cyanide <br />analysis was collected with the operator collecting a split with their own bottles.
