(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-2009-035 <br />INSPECTION DATE: 6/18/2009 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS THM <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Reclamation, Mining and Safety. 32.5 Road Gravel Pit is currently under technical review and the inspection was to <br />observe the conditions at the site prior to the operation commencing. 32.5 Road Gravel Pit is located East of 32.5 <br />Road approximately 5 miles east of Grand Junction, Colorado. The 32.5 Road Gravel Pit is a 30.25 acre, 112c <br />proposed operation. Martin Azcarraga and Jay Ingelhart both accompanied the inspector during the inspection. <br />Access to Phase 1 was not available during the inspection. However, photographs were able to be taken to <br />document the pre-mining conditions. <br />The sites were formally used for sewage lagoons for the Clifton Sanitation District. Evidence of removal of the <br />sludge from the sewer lagoons was observed during the inspection. Most of the infrastructure associated to the sewer <br />lagoons was still present during the inspection. The infrastructure observed consisted of power lines, control <br />buildings, piping, concrete manholes and the historical configuration of the lagoon ponds. <br />A large area covered with Russian Knapweed was observed in the southeast area of Phase 2 area and smaller <br />patches were observed at the south and southwest portion of Phase 2. Per Rule 3.1.10(6), Methods of weed <br />control shall be employed for all prohibited noxious weed species. All areas that are affected by the knapweed <br />should be de-vegetated and buried along with the upper 2-3 inches of soil in a slope/highwall backfill to limit <br />the spread of the noxious weed through sale or transport of construction materials from the site upon <br />commencement of mining. <br />A nest that appears to be an eagle's nest was observed at the center of the site on top of a power pole in Phase 2 <br />(South Mining Area). <br />Photos that show the conditions observed during the June 18, 2009 inspection are included at the end of this report. <br />Responses to this inspection report should be directed to Travis Marshall at the Division of Reclamation, Mining <br />and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado 81501, phone <br />no. 970-241-2042. <br />I & E Contact Address cc: <br />NAME: Martin Azcarraga ?
