(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1978-135-HR <br />INSPECTION DATE: 6-23-09 INSPECTOR'S INITIALS GRM <br />OBSERVATIONS <br />This inspection was conducted as part of the Colorado Division of Reclamation, Mining and Safety normal monitoring <br />program. The Blue Jet Mine is located east of County Road 17 off of CR 168. The inspector tried to make contact with the <br />operator several times prior to the inspection but was unsuccessful. The inspector was however accompanied by BLM Little <br />Snake personnel Jennifer Miaolo and Shawn Wiser. <br />The mine identification sign is in place and in compliance with Rule 3.1.12. The sign is located at the entrance to the site off <br />CR 168. Permit boundaries appear to be marked by t-posts but not all were visible and easily identifiable. The inspector is not <br />noting the issue as a problem but requests that the operator inspects permit boundaries and re-establishes them where <br />necessary to clearly identify the permit boundaries. <br />The Annual Report notes that %2 acre has been reseeded. Without a current map that area could not be identified. Field <br />observations do not note any areas where such activities appear to have occurred. Please provide the Division with a current <br />map showing what areas have been reseed. A map has not been supplied since at least 2005 or later. The lack of a map <br />showing current reclamation progress is noted as a problem with the corrective action(s) and correction date specified on the <br />last page of this report. <br />Noxious and/or problematic weed infestation (knapweed and thistle) was observed within the affected area. The infestation, <br />if not controlled, will most likely hinder and/or delay successful reclamation of this site and contribute to off-site spreading <br />and establishment of these weeds. A weed control plan developed in consultation with, and approved by, the appropriate <br />local weed control authority and BLM must be submitted as a technical revision to this permit. Weed infestation is noted as a <br />problem with the corrective action(s) and correction date specified on the last page of this report. Significant progress <br />toward control of on-site weeds must be demonstrated within three years to avoid potential Board action. <br />In 2006, an inspection noted "a range of debris on the mine site that needed to be cleaned up". In 2009 this inspector notes <br />some very similar if not the same issues. Field observations note the site and equipment are in severe disrepair. No visible <br />signs of mining activity were noted. Per Rule 3.1.3 all reclamation shall be carried out to completion within five (5) years of <br />completion of mining. The lack of mining activity is officially noted as of the date of this inspection. The lack of activity is <br />noted as a problem with the corrective action(s) and correction date specified on the last page of this report. The debris on <br />site is also noted as a problem with the corrective action(s) and correction date specified on the last page of this report. <br />Overall the inspector is unsure that Annual Reports on reclamation and mining activities are accurate. Corrective actions are <br />intended to document activities to show compliance with the Rules and Act.
