(Page 2)
MINE ID # OR PROSPECTING ID #: M-1 988-093
INSPECTION DATE: 64 W09 INSPECTOR=S INITIALS: RK
OBSERVATIONS
The Division conducted a monitoring inspection of sites to be inspected during the fiscal years 2008-2009. The site si a
small 112 sand and gravel operation covering 22.5 acres located in Fremont Colorado, approximately 1/2 mile West of
Florence Colorado. Site was not active at the time of the inspection. Along the East side of the permit area on the rail road
tracks, empty rail carts were observed parked. The small .5 feet of affected acreage that was at one time included as the
Energy Fuels Coal Load out area that was amended into the permit in 11/12/2004 has been completely reclaimed and
slopes were stable and moderately vegetated. No erosion features noted. Site was not active at the time of the inspection.
Present during the inspection were Miss Denise Gonzales and Mr. Phil Rieg of Fremont Paving and Redi Mix.
A small processed stockpile material was noted along the north side of the main pit. Approximately 5 to 7 acres appear to
have been disturbed at the time of the inspection. According to Denise , the current plan once the need for aggregate
arises is to commence mining along the south side of the current pit. The current high wall measuring approximately 450
feet orienting east to west. And approximately 5 to 7 feet in height.
High wall was stable and no problem noted. The current warranty for the site is $49,200.00 which is more than adequate
to cover the current disturbance. Permit sign and boundary markers properly placed.
I & E Contact Address
NAME: DENISE GOMZALES
OPERATOR: FREMONT PAVING AND REDI MIX
STREET: 839 MAC'KENZIE AVE
CITY/STATE/ZIP: CANQN CITY.
