(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1986-007 <br />INSPECTION DATE: 06/11/2009 INSPECTOR'S INITIALS: RCS <br />OBSERVATIONS <br />This was a follow-up monitoring inspection conducted by Eric Scott of DRMS for the Masonville Stone site. The inspection was <br />prompted by recurring issues with missing permit boundary markers and the relationship to the observed disturbed area noted during <br />previous inspections conducted on 12/18/08 and 2/12/09. DRMS met with Glenn Southwick at the site at approximately 0930. The <br />site entrance is located on the south side of Latimer County 29 RD approximately 100 feet west of the intersection of 29 RD and 27 <br />RD. The required postings were present at the site entrance. <br />Mr. Southwick has either re-located or replaced the boundary makers that are intended to outline the permit area as directed in the <br />previous inspection reports. However, these markers as replaced did not encompass the entire disturbed area of the site at the time of <br />the 6/11/09 inspection. One additional marker was placed on the southeast corner of the site during this inspection to further define <br />that area. At this time, the site boundaries appear to be appropriately defined. The set-aside area found in the west central portion of <br />the site should be clearly marked as a "no-mine" area to prevent inadvertent encroachment by workers. <br />In the previous inspection report the operator was directed to submit a Technical Revision with appropriate information and <br />fees to finalize the boundary adjustments for the site. <br />A Technical Revision to finalize the adjustments to the boundary of the site shall be submitted to the Division with the <br />appropriate fees within 30 days of the last inspection (7/15/09) to avoid possible violations and penalties. This revision shall <br />include a surveyed map of the permit boundary, with distance and bearings from permanent established markers - including <br />the new marker on the southeast corner of the site placed during the 6/11/09 inspection. The map should include both permit <br />boundary and disturbed area of the site, and be overlain on a base map consisting of a current air photo.
