PROBLEM: <br />INSPECTOR'S INITIALS PSH <br />The Mine Plan on file with the Division does not accurately depict the <br />conditions at the Rocky Flats site. A sedimentation pond, wash plant and <br />concrete plant were observed at the site during the inspection. The Division <br />does not have record of the sedimentation pond, wash plant and concrete <br />plant in the permit file. <br />CORRECTIVE ACTION: <br />Submit a technical revision to include the items listed below to update the <br />mine plan for the Rocky Flats Pit site: <br />1. Include the concrete plant, wash plant and sedimentation pond to the <br />permit or backfill the sedimentation pond, end the washing process <br />and provided evidence the concrete batch plant has been removed <br />from the site. <br />a. The technical revision must include the required $216 fee, all <br />required county or local government permits, including but not <br />limited to an APENS permit, storm water and spill control plan <br />from the Colorado Department of Public Health and <br />Environment and all applicable permit exhibits. <br />2. Demonstrate the source of the wash plant water. <br />a. If the source is groundwater, please provide the Division with <br />proof of a well permit and substitute water supply plan from <br />the Office of State Engineer. If the source of is storm water, <br />please provide the Division with proof of the right to retain the <br />water on site. <br />3. Demonstrate the right from the Office of State Engineer to retain storm <br />water in the pit. <br />4. Submit a revised Mine Plan Map that accurately depicts the Rocky Flats <br />site including the internal road configuration, Church and McKay ditch <br />locations, stockpile locations and the locations of the concrete batch, <br />processing facility and wash plant locations. <br />CORRECTIVE DATE <br />7/13/09 <br />PB or PV:PB <br />TOPIC(S):AR <br />The technical revision should address the construction, operation and <br />reclamation of the unpermitted items. All applicable permit exhibits including <br />maps and reclamation costs must be revised by the operator.
