4 <br />APPLICATION REQUIREMENTS <br />Type or print clearly in the spaces provided below, the appropriate page number(s) of the permit <br />application where a narrative discussion for each information category may be found. Where a map <br />has been used to supplement or clarify the narrative, indicate in the space provided below, where the <br />appropriate map(s) may be found in the application. Please note that certain maps related to specific <br />information categories are required for an application to be considered complete. Where maps are <br />required, a title description of the applicable subject matter to be included, the applicable regulatory <br />citation describing the required information to be presented, and an asterisk (*) appear in the <br />appropriate space below. <br />Each permit application must provide appropriate information which minimally addresses each of the <br />following requirements.
