ORIGINAL <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-19Rn-9aa <br />INSPECTION DATE: 5/91,409 INSPECTORS INITIALS: RK- <br />OBSERVATIONS <br />The Division conducted a construction progress inspection of the Amendment # 9 Mine Life <br />Expansion areas. Present during the inspection was Mr. Tim Comer AND Mr. Tom Shepherd of <br />CC&V . <br />Mr. Shepherd explained the areas under construction. The tornado shat backfilling was <br />continuing. The fickler wall for the Loud out Bin relocation is now completed. Haul road <br />leading to the LOB was being cut at the time of the inspection. <br />The clay borrow area that was used to generate the clay that will be needed for the Phase <br />V leach pad has been graded and sloped. <br />Earth moving was continuing along the old Arequa Gulch reclaimed overburden storage area <br />that is going to be the new Phase V leach pad area. <br />The new storm water sediment pond south east of the current LOB has been lined with clay <br />to limit infiltration. <br />Storm water control BMPS appear to be in good repair. <br />Power transfer poles were being erected along the Phase IV C perimeter area. <br />Inspected the LDS 11 area that was partially affected during the power pole and gas line <br />placement operation.
