11111 <br />• The failure, which occurred on December 2, 2008, is about 14.8 acres in area. <br />About one half of the failed area had been previously reclaimed. The easily visible <br />scarp of the displaced slide mass is up to about 70 feet high and is about 1,300 feet <br />long (Fig. 1, photograph). The northern edge of the slide mass is somewhat <br />obscured by the rubble nature of the slide mass and the adjacent ragged un- <br />reclaimed slope below the north peak. <br />PREVIOUS INVESTIGATIONS <br />The quarry area was investigated by Dames and Moore for the 1977 mining <br />permit. Dames and Moore produced an Evaluation of Aggregate Limestone <br />Reserves of the Pikeview Quarry (Job No. 08139-006-14) and a Geologic Mapping <br />Project Pikeview Quarry (Job No. 08139-005-14). The reserve evaluation report <br />included four boring logs, but not information on the locations or elevations of the <br />borings. The copies of both the reports we were provided did not include any <br />• figures or maps with geology or boring locations. <br />The Application for Permit Amendment, Pikeview Quarry, Permit M-77-211, <br />dated May 1994, prepared by Mark Heifner with assistance from Obering, Wurth & <br />Associates and CTL I Thompson, included some information on the geology and <br />subsurface conditions. <br />Banks and Gesso, LLC prepared a Construction Material, Regular 112 Permit <br />Application Amendment, Pikeview Quarry Amendment Two, dated September 22, <br />2000. This application also included limited subsurface information, rock <br />mechanics slope design criteria, a survey of the site, and a revised reclamation <br />plan. <br />Mark Heffner (Southwestern Ecological Services) and B.J. Fuller (Castle <br />Concrete/Transit Mix) prepared an Integrated Mining Plan in February of 2003. The <br />purpose of the document was to integrate a mining operation plan with the <br />reclamation plan in order to minimize the amount of post-mining reclamation that <br />• would need to be performed. <br />TRANSIT MIX AGGREGATES 5 <br />PIKEVIEW QUARRY SLOPE <br />CTLIT PROJECT NO.
