MANAGEMENT <br />RECEIVED <br />MAY 2 2 2009 <br />GRIND JUNG I i(r. c iclb OFFICE <br />DIVI6ION OF <br />RECLAMATION MINING 8 SAFETY <br />The DRMS's inspection report dated May 13, 2009 noted a significant crack that <br />has opened up at one of the portal entries. On behalf of CAM Mining, LLC, <br />following is a plan to permanently repair the portal entry. The repairs will be <br />complete by the end of June 2009 or 30-days after plan approval whichever is <br />later. <br />Repair Plan: <br />Versa Foam System 15 will be spayed into the portal void. The outby section of <br />portal void will be filled with foam. CAM expects to use five to ten packs of foam. <br />The foam will provide a back stop for dirt fill. The foam will be plated with nine to <br />twelve inches of dirt and rock. The specifications of the foam are as follows: <br />Yield 16.5 cubic feet per pack <br />Strength 16.7 psi, 2405 psf <br />Weight 40 pounds per pack <br />The foam is surprisingly strong.
