(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-2002-053 <br />INSPECTION DATE: 5/27/09 INSPECTOR'S INITIALS: -ECS <br />OBSERVATIONS <br />This was a routine monitoring inspection for an active 110c sand and gravel operation conducted by Eric Scott <br />of DRMS for the Kurtzer Pit site. DRMS met with site owner/operator John Kurtzer and son Ryan Kurtzer on <br />site at approximately 1330 and toured the site with them. The site was generally in good condition, however <br />there were a few items noted that will require either on-site corrections or revisions to the approved permit. <br />The required site boundary markers need to be installed as no boundary markers were present. If the original <br />extent/configuration of the site is unable to be determined from the site file, DRMS will have to assume that all <br />visibly disturbed areas are within the permit boundaries up to the original 9.9 permitted acres. This would make <br />all disturbed areas subject to the approved reclamation standards of the permit. <br />Several weeds were noted that will have to be eliminated prior to release, such as mustard, mullein etc., but they <br />are still very manageable at this point. <br />The approved mining and reclamation plans on file with DRMS currently state that slopes at the site will be <br />maintained at 3:1 or flatter during mining, and for final reclamation. Several areas were noted that will have to <br />be resloped to meet this standard and prior to release. <br />There was some discussion with the operator during the inspection about reclaiming the pit for final release, and <br />the status of the existing material stockpiles on site. DRMS policy for stockpiled material is that if the <br />stockpiles are for the owner/operator's personal use (i.e. NOT for sale), then they may remain on site after the <br />pit is reclaimed and released. All other conditions of the approved reclamation plan (sloping, topsoil, seeding <br />and re-vegetation) would still have to be met prior to the final release of the permit. <br />In addition to the inspection topics, it was noted during the file review that an adequate annual report for this <br />site has not been submitted for the past several years. Operators are required to submit an updated site map and <br />a written summary of the past year's activity at the site with their annual fees. <br />I & E Contact Address <br />John R.
