III. COMMENTS -COMPLIANCE <br /> Below are comments on the inspection. The comments include discussion of observations made <br /> during the inspection. Comments also describe any enforcement actions taken during the inspection <br /> and the facts or evidence supporting the enforcement action. <br /> This was a complete inspection of the Munger Canyon Mine conducted by Mike Boulay of the DRMS <br /> Grand Junction Field Office on May 13, 2009. I was not accompanied by an operator representative <br /> during the inspection. Weather conditions were warm, clear and dry. Ground conditions were dry. <br /> The majority of the mine site is reclaimed and there was no activity at the site during this inspection. <br /> Several items were identified as needing to be repaired during the Phase I bond release inspection <br /> conducted on 12/03/08 at the Munger Canyon Mine site. Please refer to the bond release inspection <br /> report dated 12/16/08 for more details of these items and locations. The repair and maintenance was to <br /> be conducted as soon as weather permitted full access to the site. Gary Isaac of CAM Mining LLC <br /> informed me that these repairs had been completed, so we scheduled a complete inspection of the site <br /> to view these areas. During this complete inspection on May 13th, I noted repairs had not been <br /> satisfactorily completed at a few locations. These include three silt fences one each at the mine bench, <br /> Channel H, and at the topsoil pile. Also the crack/slough that has opened up at the location of one of <br /> the portal entries on the mine bench area has not been backfilled and stabilized as requested following <br /> the bond release inspection. These locations are described below and photographs are provided. <br /> Availability of Records <br /> The availability of records was checked at the Munger Mine office on the afternoon of May 13th <br /> Several items needed to be updated in the records file as described in the last complete inspection <br /> report conducted on March 9, 2009. All of these items were updated as requested. All required <br /> records were on file and available for review during this complete inspection and no problems were <br /> identified with the records review. <br /> Roads <br /> The access road to the mine site was dry and the road was generally in good condition. No problems <br /> were noted along its entire length up to the former portal bench access road. <br /> Hydrologic Balance <br /> East Salt Creek was flowing with turbid water due to snow melt runoff. Ditches and culverts <br /> associated with the reclaimed waste disposal site, cover soil stockpile site, and the topsoil stockpile <br /> locations were in good condition and maintained. The waste disposal area sediment pond was dry. <br /> The inlet and discharge structures were stable. <br /> The former portal bench access road was inspected. Overall the reclamation looks very good and there <br /> has been minimal erosion since the reclamation was completed in 2007. All of the permanent drainage <br />
