follow-up information needed to assess whether the actions successfully mitigated the unsafe <br />practice. <br />The information provided on the forms is also added to the AMSO BBS database where the <br />information is used to update the SHP as necessary and track observed. at-risk behaviors at <br />project sites. The information compiled in the database is reviewed to identify any trends or <br />potential trends in at-risk behavior. Potential trends are monitored to determine if actual trends <br />are suggested in the data. Once a trend is identified, an action plan is developed to address the <br />at-risk behavior or practice. <br />The AMSO BBS Observation Process provides a constructive tool for maintaining and <br />continuously improving a safe work environment. The objectives of the process are: <br />• Demonstrate that an actionable process is in place for continuous safety improvement. <br />• Ensure employees are properly trained and encouraged to observe each others actions, <br />recognize and properly document unsafe practices, openly provide feedback/input <br />regarding corrective measures. <br />• Track at-risk behaviors observed at AMSO work sites and update the SHP as necessary.. 0 <br />• Monitor for trends in at-risk behaviors and develop action plans to address any <br />identified trends. <br />• Ensure actions are tracked through to completion. <br />3.1.4 Chronic Safety Offenders <br />Anonymity will be enforced for first-time offenders. However, if an employee exhibits serious <br />and chronic safety issues, coworkers are encouraged to notify the project supervisor of their <br />observation before any other actions are taken. Employee-employee public or private <br />confrontations are prohibited, as this sort of behavior may also incite personal conflict and <br />cause safety issues within the working crew itself. <br />Once notified, the project supervisor will observe the indicated employee for evidence of chronic <br />safety issues and take the appropriate actions to ensure that the unsafe behavior is corrected and <br />eradicated. <br />•
