MODIFICATION 03-2009 <br />Exhibit C - AMSO, LLC Notice of Intent Supplemental Information <br />DRILL PAD MWP-2 <br />WS (Wasatch Monitor Weli (See Figure C.8-Amend 03-2009 in this section) <br />• Drill 17" hole a tninimuni of 40 feet or until at least 5 feet of competent bedrock <br />is encountered. <br />Rini 113/4", 47#, J-55 Surface Casing. <br />• Cement with "Neat Cement" to surface. <br />• Rig up diverter and wait on cement a minimum of 4 hours. <br />• Drill out thrli the surface casing with 10 5/8" diameter PDC bit. Advance drill <br />hole to -2380 feet and circulate a minimum of 30 minutes. Run geophysical logs. <br />• Run 5 1/2", 15.5#, J-55 casing with a guide/float shoe. Mix and pump Type I or 11 <br />Portland cement with 2% via trernie in multiple lifts. Wait on cement for a <br />minimum of 12 hours. <br />• Trip in the hole with nominal 4 5/8" drill bit and drill cement and float shoe. <br />• Air drill to a Total Depth of 2480 feet. <br />• Circulate the hole for 30 minutes until formation water is observed. <br />• <br />0 <br />12
