(Page 3) <br />MINE ID # OR PROSPECTING ID # M-2001-023 <br />INSPECTION DATE 05/18/09 <br />INSPECTOR'S INITIALS PSH <br />The Routt County meeting to decide on the Special Use Permit for the Camilletti Milner Pit #2 expansion is <br />scheduled for May 27, 2009. Precision will request an exemption of the required 25 acre maximum <br />disturbance area to increase the disturbed area to 35 areas. The increased acreage would allow for safe and <br />reasonable circulation of equipment and adequate space for processing facilities, stockpiles of multiple <br />materials and circulation of haul trucks in and out of the pit. The site is currently bonded for 23 acres of <br />disturbance by the Division. Technical Revision No. 1 (TR-01), approved on September 5, 2001, allowed a two <br />phase mining plan with an incremental bond of $100,000. Each mining phase is approved for 18 acres of <br />active disturbance and 5 acres of moderate disturbance at any one time. The proposed amendment will <br />require a change in the financial warranty for the site. Precision Excavating will submit updated phased <br />reclamation costs to the Division after Routt County approval of the Special Use Permit. <br />The Operator has not notified the Colorado Office of State Engineer of the proposed change to the mining <br />plan. The operator will be required to obtain a revised well permit and substitute water supply plan from the <br />SEO before exposing groundwater in the amendment site. <br />Kelly Colfer with Western Bionomics LLC identified approximately 2 acres of wetlands in the amendment <br />area. The wetlands were observed during the inspection. Precision intends to mine through the wetlands <br />and re-establish 2 acres of wetlands off-site. The operator must commit to not disturbing the wetlands prior <br />to receiving a 404 permit for the Army Corps of Engineers. <br />The amendment area will be connected to the existing pit by bridge and conveyor. The applicant agreed to <br />submit construction details for the proposed bridge and conveyor structure over Trout Creek to the Division <br />prior to construction. <br />The Operator estimates the existing Pit #2 contains 2-3 years of gravel resources before mining would <br />commence on the amendment area. The operator has agreed to a "slot" mining plan near the Cottonwood <br />trees in the amendment area to limit the stress on the trees. <br />The Division will send an adequacy review letter to the operator requesting additional information under <br />separate cover. Mr. Zehner requested information on using imported fill in the amendment site to created <br />the pond shorelines.
