(Page 4) <br />MINE ID # OR PROSPECTING ID # M-1980-027 <br />INSPECTION DATE 04/14/09 <br />INSPECTOR'S INITIALS PSH <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1. PROBLEM: <br />Sedimentation ponds associated with a wash plant and a concrete <br />batch plant were observed during the inspection. The Division <br />does not have documentation of these items in the permit file. <br />CORRECTIVE ACTION: <br />The operator must, by the corrective date, provide documentation <br />of the items above OR submit a technical revision to include the <br />sedimentation ponds, wash plant and concrete batch plant in the <br />permit and receive approval within the statutory timeframe. The <br />technical revision should address the construction, operation <br />and reclamation of the unpermitted items. All applicable permit <br />exhibits including maps and reclamation costs must be revised by <br />the operator. <br />3.
