Cattany <br />Division Director <br />Natural Resource Trustee <br />Re: CBC Gravel Pit, File No. M-20097030, New Application for 110c Permit, Application Incomplete. <br />Dear Mr. Cressler, <br />I have finished the first review of the 110c permit application named above, in order to ensure whether it is <br />complete or not. At this time, there are several items that appear to be missing or incorrect, and until they <br />are provided or revised, the application cannot be considered "complete," and this office cannot begin its <br />technical adequacy review. The items to address are identified by the application or exhibit name, and may <br />refer to the Construction Materials Rules and Regulations. Please respond to the items listed below: <br />Application - Applicant/operator or company name. <br />The name to be used must be one that is registered with the Colorado Secretary of State and in good <br />standing. CBCI Gravel is not registered, and the application form must be revised to reflect an applicant's <br />name that is registered. We spoke on the telephone, and agreed that your registered company name, Coal <br />Bed Creek Construction, Inc., may be substituted on the correct pages of the revised form. I have received <br />two copies of the revised form that you sent, and find that they do, in fact, now include a correct operator <br />name and updated notary seal and date. However, the new applicant name is a corporation, and there is no <br />corporate seal affixed to the signature page. Please provide two copies of the signature page with the <br />required corporate seal, or if there is no seal, please write "no seal" or draw a seal. <br />Exhibit E - Mining and Reclamation Maps (Rule 6.3.5). <br />The application only included information concerning easements, but no maps were submitted. Please see <br />Rule 6.2 for the applicable map standards, and Rule 6.3.5 for description of the required maps. You must <br />provide a map showing the mining plan and a map showing the condition of the site after reclamation has <br />occurred. The details depicted on the maps must support the narrative descriptions in Exhibit C and D. <br />Among the other map requirements, each map must be drawn at a proper scale, by a person qualified to <br />prepare such maps, and be signed and dated. <br />Exhibit G - Proof of Legal Right to Enter (Rule 6.3.7) <br />Please provide one of the acceptable forms of proof of the right to enter upon the proposed permitted land <br />and performing mining and reclamation.
