(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2006-075 <br />INSPECTION DATE 04/16/09 INSPECTOR'S INITIALS PSH <br />OBSERVATIONS <br />The Overlook Mine, File No. M-2006-075, was inspected by Peter Hays with the Division of <br />Reclamation, Mining and Safety (Division) as at the request of Mr. Ken Evans a <br />representative of Granby Realty Holdings, LLC. <br />A meeting was held at the Granby Ranch office located at 63331 US Highway 40 in Granby, <br />Colorado to discuss the requirements of the Division for Granby Realty Holdings, LLC to <br />add an asphalt plant to the Overlook Mine permit. Mr. Kyle Harris, Senior Vice President <br />of Development and Mr. Ken Evans were present during the meeting. Mr. Harris stated the <br />asphalt plant would produce asphalt to be used within the Granby Ranch development only. <br />Granby Realty Holdings, LLC would be required to submit a technical revision per Rule 1.9 <br />to add an asphalt plant to the Overlook Mine permit. A copy of the Rule is attached for <br />reference. The Division requires the technical revision application to include the <br />following information at a mininium: the $216 technical revision fee, a revised mine plan <br />map indicating the location of asphalt plant, proof of the Grand County special use permit <br />allowing the asphalt plant, the required Air Pollutant Emission Notice (APEN), Hazardous <br />Material and Spill Control permits from the Colorado Department of Public Health and <br />Environment. <br />The Division encourages Granby Realty Holdings, LLC to contact the Division with <br />additional questions. <br />I & E Contact Address <br /> ?
