TECHNICAL REVISION <br />Pikeview Quarry <br />M-77-211 <br />Castle Concrete Company <br />April 2009 <br /> <br />RECEIVED <br />APR 0 3 2009 <br />pwision at Neoamatiorl, <br />Wing and safety <br />PURPOSE OF REVISION: To provide a plan to monitor movement of the slide area, <br />overhanging limestone, tension cracks, and areas immediately north and south of the <br />slide. <br />REASON FOR REVISION: This revision is in response to a Mineral Program <br />Inspection Report section Corrective Actions, page 3, item 2, dated 12/5 & 11/08 and <br />2/10/09. <br />DOCUMENTS: Enclosed is a scanned quotation and purchase order from Leica <br />Geosystems for monitoring equipment. There is also an aerial photo of the Pikeview <br />quarry. <br />Document 1: Leica Geosystems quotation for an automated motorized total <br />station and related equipment. <br />Document 2: Aerial photo of the Pikeview Quarry with a designated area for the <br />monitoring equipment. <br />CORRECTIVE ACTION PLAN <br />GENERAL COMMENTS REGRDING THE SLIDE AREA: A slide occurred on <br />December 2, 2008 at the Pikeview Quarry. The area encompassed both an area that is <br />being actively mined along with an area that is reclaimed. Scarping has occurred into the <br />area designated as the layback which includes an area of National Forest Service <br />property. Geotechnical investigations are ongoing as to the cause of the slide. <br />MONITORING PLAN <br />Lieca Geosystems is an industry leading in monitoring equipment. Lieca engineers were <br />provided relevant information on the slide and concluded a total robotic station would be <br />the best choice for the application.
