PERMANENT MAN-MADE STRUCTURES EXHIBIT S <br />The following is an inventory of man-made structures within 200 feet of the disturbed area. All <br />of these structures are shown on Maps C-1, C-2, and F. The landowner boundaries are shown on <br />Map C-1. The damage waivers for these structures are included in this section and are signed and <br />notarized by the operator. We were unable to get a signed version back from CDOT and Qwest <br />therefore an evaluation of the stability of the slope is included Exhibit U to satisfy Rule 6.4.19 <br />(b). <br />1. Highway 160 and associated culverts, owned by Colorado Department of Transportation. <br />2. Buried communication line along highway 160, owned by Qwest. <br />Simmons Pit, March 2009 S-1
