The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS and CORRECTIVE ACTIONS <br />PROBLEM or POSSIBLE VIOLATION: PBJ(problem) CORRECTION DATE: October 10, 2006 <br />TOPIC(S): SM (Signs and Markers) i <br />DESCRIPTION: The sign posted at the entrance to the site does not meet the criteria presented in Rule <br />3.1.12(1). <br />CORRECTIVE ACTION NARRATIVE: Post a sign at the entrance to the site that meets the criteria presented <br />in Rule 3.1.12(3) which are as follows: a) name of operator b) statement that a permit has been issued by the <br />Colorado Mined Land Reclamation Board and c) permit number.
