LIST OF OTHER PERMITS AND <br />LICENSES REQUIRED EXHIBIT F <br />The following permits are needed for the site: <br />1) County Special Use - A full Special Use Permit is needed with the County Planning and <br />County Commissioner's office. The operator will be submitting a new application for this permit <br />concurrently with the DRMS amendment application. <br />2) APEN - A fugitive air emissions permit may not be needed for this site because the annual <br />production will be less than 70,000 tons per year. An APEN will be submitted and CDPHE will <br />decide if a permit is necessary. Air emissions permits will be in place for all portable equipment <br />utilized on site which requires a permit. <br />3) A Montrose County Access Permit will be required for vehicles entering Clear Fork Road. <br />4) A Spill Prevention Control and Countermeasure (SPCC) Plan is needed for the Mott pit <br />mining operation. No spilled material will be allowed to leave the site and all tanks will have <br />secondary containment. In addition, the portable crusher and asphalt plant that will be on site <br />from time to time each have their own separate SPCC Plans. These plants may bring tanks to the <br />site, but all other times there will not be a tank on site. <br />5) No USCOE 404 permit will be necessary because no wetlands or waters of the U.S. will be <br />disturbed. <br />Mottpit, March 2009 F-1
