TERMS & CONDITIONS •
I. This permit is issued in conlunction ++•ith Permit Ns 399058. Submittal for design shall include considerations for
all of these permits. (only one submittal required) If there are any questions regarding this permit. please contact

?. The Permittee shall design and submit (or approval a left turn deceleration lane 16 feet in width m conformance
with section 4 of [he State Highway Access Code, 2 CCR 601
3. Design of this access with auxiliary lanes shall include, at a minimum, I'fe-inch overlaiy.
4. A Notice to Proceed, CDOT Form 1265 is required before beginning the construction of the access or smy activity
within the highway right-ot-way. All submittals, documents, plans and other items that must be completed, shall
be submitted and approved before a Notice to Proceed will be issued. Contact the Department (or local issuing
authority) for the Notice to Proceed.
5. The following items are required before a Notice to Proceed will be issued:
(a) Construcnon Plans Stamped by a Colorado Registered Professional Engineer.
(b) Certificate of Insurance Liability as per Section 2.3(1 I)(h) of the Access Code.
(c) Traffic Control Plan
6. The Permittee shall refer to all additional standard requirements on the back of [his permit and any enclosed
additional terms, conditions, exhibits and noted attachments.
7. This Permit is issued in accordance with [he State Highway Access Code (2 CCR 601-] ), and is based in part upon
the information submitted by the Permittee. This permit is only for the use and purpose stated in the Application
and Permit. Any changes in traffic volumes or type, drainage, or other operational aspects may render this perrziit
void, requiring a new permit to be applied for based upon existing and anticipated future conditions.
8. Upon completion of the access, the applicant shall notify [he Access Manager by certified mail within 10 days :tt:

9. Nothing in this permit shall prohibit the chief engineer from exercising the right granted in CRS 43-3-102
Including but not limited to restricting left hand turns by construction of physical medial separations.
10. All costs associated with the installation of traffic lights for this access are [he responsibility of the Permittee. This
includes but is not limited to the design, consvuction, utility relocation, testing of materials and inspection.
11. Water, sanitary, sewer, gas, electrical, communication, landscaping, and telephone installations will require
individual additional permits.
12. The Permittee is responsible for obtaining any necessary additional federal, state andJor City/County permits or
clearances required for construction of the access. Approval of this access permit does not constitute verification
of this action by the Permittee.
13. Any work within State Hiehway right-of-way shall begin after 8:30 A.M. and all work and equipment shall be off
the highway BEFORE 3:30 P.M. each day.
14. No highway lane closures or one-way traffic will be allowed.
I5. No work will be allowed at night. Saturdays, Sundays and legal holidays without prior authorization from the
Department. The Department may also restrict work within the State Highway right-o(-way during adverse
weather conditions.
16. It is the responsibility of the Permittee to prevent all livestock from entering the State Highway right-o(-way at this
access location. Any livestock that does enter the highway right-of-way shall be the sole responsibility of the •
Permittee.
