TERMS & CONDITIONS
1. This permit is issued in conjunction with Permit N~ 399059. Submittal for design shall include considerations for

7. Desiun of this access with auxiliary lanes shall include, at a minimum. 1'h-inch overlay.
4. A Notice to Proceed. CDOT Form 1265 is required before beginning the construction o(the access or any activity
within the highway right-of-way. All submittals, documents, plans and other items that must be completed. shall
br submitted and approved before a Notice to Proceed will be issued. Contact the Depannient (or local issuins
authorih•) for the Notce w Proceed.
5, The follotiing items are required before a Notice to Proceed will be issued:
(a) Construction Plans Stamped by a Colorado Registered Professional Engineer.
(b) Certificate of Insurance Liability as per Section 2.7(1 I)(h) of the Access Code.
(c) Traffic Control Plan
6. The Permittee shall refer to all additional standard requirements on the back of this permit and any enclosed
additional terms, conditions, ezhibits and noted attachments.
7. This Permit is issued in accordance with the State Highwa}' Access Code (2 CCR 601-I), and is based in part upon
the information submitted b}' the Permittee. This permit is only for the use and purpose stated in the Application
and Permit. Any changes in traffic volumes or type, drainage, or other operational aspects may render this permit
void, requiring a new permit to he applied for based upon existing and anticipared future conditions.
8. Upon completion of the access, the applicant shall notify the Access Manager by certified mail within 10 days at: .

9. Nothing in this permit shall prohibit the chief engineer from exercising the right granted in CRS 93-3-102
Including but not limited to restricting left hand turns by construction of physical medial separations.
10. All costs associated with the installation of traffic lights for this access are the responsibility of the Permittee. This
includes but is not limited to the design, construction, utility relocation, testing of materials and inspection.
I I. Water, sanitary, sewer, gas, electrical, communication, landscaping, and telephone installations will require
individual additional permits.
12. The Permittee is responsible for obtaining an}' necessary additional federal, state and/or City/County permits or
clearances required for construction of the access. Approval o(this access permit does not constitute verification
of this action by the Permittee.
13. Any work within State Highway right-of-way shall begin after 8:30 A.M. and all work and equipment shall be off
the highway BEFORE 3:70 P.M. each day.
14. No highway lane closures or one-u•ay traffic will he allowed.
I5. No work will be allowed at night, Sawrdays, Sundays and legal holidays without prior authorization from the
Department. The Department may also restrict work within the State Highway right-of-way during adverse
weather conditions.
16. ll is the responsibility of the Permittee to prevent all livestock from entering the State Highway right-of-way at this .
access location. Any livestock that does enter the highway right-o(-way shall be the sole responsibility of the
Permittee.
