A map of where the proposed property is located within Park <br />County. <br />(10) The property must be clearly identified with the address and posted according <br />to the Park County address requirements. (requirements attached) <br />(11) Written description of proposed use and intended development. <br />(12) A list of names anti +rtailing addresses of all owners of at JacPnt property to the <br />property subject to the proposed variance, this information appears of record <br />with the Park County Assessor's Office. <br />(131) Map of current conditions. lit a minimum., the :nap must show all, of the <br />following information: <br />a. The current zone district(s) of the property to be rezoned and the adjacent <br />properties. <br />b. A survey map showing the boundaries of the property to be rezoned in <br />acres. <br />c. A description of the existing uses on the property and on adjacent <br />properties. <br />(14) Applicant must provide a Proposed Development Plan describing in mapped <br />form and/or text the existing and proposed physical improvements of the <br />property and their locations. <br />For properties currently within a dining Zone District, the additional materials <br />must be included in a complete application: <br />(15) The patent, plat, field notes and connected sheets of the mining claim of the <br />subject property and land status maps of property located within a one mile <br />radius of the subject property's boundaries. These documents may be <br />obtained from the Bureau of Land Management offices located at 2850 <br />Young field Street, Lakewood, Colorado. <br />(16) A copy of the U.S.G.S. topographic quadrangle (1:24,0w scale) of the area <br />with the subject property drawn on the map. The building envelope must be <br />at an elevation of not more than 11,500 feet. <br />(17) i ne application must include a geo-technical report prepared by a qualified <br />geo-technical professional utilizing the information and guidelines contained <br />in "Guidelines and Criteria for Identification and Land Use Controls of <br />Geological Hazard and Mineral Resource Areas, " Colorado Geological <br />Survey Publication 6, or the updated version of this publication. The geo- <br />technical report must be accompanied by a statement of the engineer's <br />qualifications relative to geology, mine economics and geological hazards.
