ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M- I 9A0-944 <br />INSPECTION DATE: 9/1/,/02 INSPECTORS INITIALS: R_ <br />OBSERVATIONS <br />The Davison conducted a monthly construction and monitoring inspection of the site on <br />2/16/09. Since the Mine Life Extension Amendment # 9 was approved, earth moving work for <br />the load out bin move, sub station, clay borrow area, visual berm for Gold Field and power <br />pole replacement pads, were progressing at the time of the inspection. Mr. Tom Shepherd, <br />the project manger for the construction phase was present to explain the progress. He <br />informed the Division that all the identified under ground workings where the load out bin <br />will be moved will be remiadted as approved in the reclamation plan. <br />DRMS requested to be present when they start backfilling some of the larger under ground <br />workings, when possible. He pointed on amp the # of identified under ground workings where <br />the load out bin load DBMS also requested the operator to start providing weekly <br />construction QA/QC reports, which he agreed to commence beginning the week of this <br />inspection date. With Gary Goodrich and Tim Comer, Tom Shepherd drove us to the <br />construction areas. All of the above sites were active at the time of the inspection. <br />Prior to the construction meeting and inspection, with Mark Vanoni of CC&V, THE Valley <br />leach facility was inspected. The following readings were recorded from the Phase I,II,II <br />&IV, HIGH AND LOW Volume solution collection transducers. <br />Phase I High Volume solution collection transducer # 1 was recorded at 40.8 # 2 was at <br />45.6 and # 3 was at 45.6 Pizometer for the pond was recorded at 55.5, DRMS requested the <br />operator to verify the readings from the transducer to make sure they correlate with what <br />is recorded in the piezometer. Operator agreed to verify the readings as soon as possible. <br />Low volume transducer # 1 was recorded .32 and # 2 was at .41 <br />Phase II High Volume solution collection transducer # 4 was recorded at 34.6 # 5 was at <br />29,5 and # 6 was at 34.5. Piezometer for the pond was at 34.5 feet. Low volume solution <br />collection transducer # 1 was at .13 and # 2 was at .30 <br />Phase IV High volume solution collection transducer # 7 WAS RECORDED AT 42.4 # 8 WAS AT <br />41.5 #9 WAS AT 42.7 and # 10 was at 43.0. Low volume solution collection transducer # 1 <br />was at 17.5 and # 2 was at 7.1 inches of hydrostatic head. <br />Inspected the Cresson mine, it was active at the time of the inspection. Storm water BMPS <br />in good repair. Ames construction has placed their equipment storage are South East of the <br />main Cresson mine, and their trailer is located east of Phase I, west of the main gate to <br />the valley leach facility. <br />Inspected the Phase IV valley leach drainage enhanced area, no problem noted. <br />Inspected the external pond, it was holding about the same amount of partially frozen <br />water. Low volume solution collection transducer for the pond were recorded at 12.3 for # <br />1 and # 2 was at 13.7 <br />Inspected the under drain area. Both the 4 inch pipe and south under drain were <br />discharging at .28 and 1 gpm respectively. All lower ponds were holding about the same <br />amount of snow covered partially frozen water, no problem noted. Inspected AG 1.5, it was <br />dry. <br />I & E Contact Address <br />NAME: MR.
