NOTICE OF INTENT NO. : C-81-013 MR-05 ADDRESS: 14300 HIGHWAY 12 PROJECT NAME:INSTALLATION OF DEGAS WELLS WESTON, COLORADO 81091 COUNTY: LAS ANIMAS DATE: DECEMBER 19, 1996 This report is filed in accordance with Rule 2.02.6(3)(b) of the Regulations of the Colorado Mined Land Reclention Board for Coal Mining adopted in accordance with the Colorado Surface Coal Mining Act, C.R.S. 34-33-101 at sea. The report is to be submitted to the Division within 30 days after the end o? W period of exploration specified in the approved Notice of Intent, as amended. i. Attach a map showing: a.) The final locations of ail holes drilled during this project; b. The locations of all roads modified or constructed for the project; and c. I All areas significantly disturbed by the operator. 2. Heve all disturbed areas, including drill holes, been reclaimed in accordance with the approved plan and the above-referenced regulations? NO 3, Were any newly constructed protect roads retained for the landowner? YES If yes, a) attach a letter from the landowner that grants consent, b) Indio the road location on a map, and c) describe the approximate specifications of the road (width, length, surface material used). 4. Did ally drill holes experience surface artesian flow conditions? NO If yes, locate these holes on a no and describe sealing and plugging procedures that were used. S. were any drill holes converted to a water well? NO If yes, list the State Engineer's Well Permit Number and name and address of the user.
