(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2001-069 <br />INSPECTION DATE 1/30/09 <br />INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of Construction Materials 112c permits. The operator <br />was contacted about the scheduled inspection. The operator's representative named on page one was present during the <br />inspection. <br />The required permit ID sign and the boundary markers were observed. The site was active at the time of the inspection. All <br />activity was within the boundary. <br />The operator was excavating material from the SW pit and hauling it to the processing equipment in the central area. There <br />was a small amount of ice on the floor of the SW pit. Processed aggregate was being stockpiled between the processing area <br />and the asphalt batching area. No batching was occurring during the inspection (due to winter temperatures). There were <br />stockpiles of cold mix asphalt and reject asphalt north of the batching equipment. These are regularly used, either hauled <br />offsite or re-processed. <br />No mining was occurring in the SE pit. There was a layer of ice on the SE pit floor, presently low but there were signs of a <br />higher water level perhaps before winter. This indicates that water subs into the pit seasonally. The operator stated that this <br />enters from the large ditch to the south, and was considered by the ditch company to be a recharge pit, and which was <br />approved by the Division of Water Resources. If so, then the seasonal water in the pit might not constitute groundwater and it <br />might not become a problem. However, the operator should provide adequate documentation to this inspector to confirm this. <br />A large stockpile of reject material was in the SE pit, that will be used to reduce the E highwall of the SE pit by backfilling. The <br />topsoil stockpile for the SE pit was along the top of the E wall. <br />The operator has not disturbed the ditch that crosses the site, between the pits and the processing/stockpiling areas. The old <br />farm building in the N half that existed at the time of the permit issuance has not been disturbed either. <br />The scale and scale house were noted along the north entrance road. The inactive batching equipment included a diesel fuel <br />tank that did not have impermeable secondary containment below it. The tank was empty and not being used at the time of the <br />inspection, and this is not noted as a problem. But the operator must ensure that this tank, and all tanks or vessels used for <br />Fluids onsite, have impermeable secondary installed beneath them. This is important to protect the topsoil and the <br />groundwater. <br />The bond was recalculated in 2006, and will be due for recalculation again in 2010. It is presently considered to be adequate. <br />No further items were observed during the inspection.
