(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1991-083 <br />INSPECTION DATE 01/16/09 INSPECTOR'S INITIALS PSH <br />OBSERVATIONS <br />The Ula Pit, File No. M-1991-083, was inspected by Peter Hays with the Division of <br />Reclamation, Mining and Safety (Division) as part of the Division's 4-year monitoring <br />inspection program. Mr. Kent Beach with Seifert Construction was present during the <br />inspection. <br />The Ula Pit is located approximately 3 miles west of Westcliffe, Colorado. The Division <br />approved an 110c to 112c reclamation permit conversion on September 7, 2004. The site is <br />permitted for 40 acres with a maximum allowed disturbance of 15 acres at a time. The pit <br />was previously inspected by the Division on August 30, 2004. <br />The Ula Pit has three permit conditions agreed on during the conversion application <br />process: the maximum allowed depth is 45 feet, the size of the excavation area is limited <br />to 15 acres (5 to 10 excavated acres at any given time and 5 acres devoted to product <br />stockpile, topsoil and related processing equipment) and no asphalt or concrete plant, <br />temporary or permanent, will be located at the mine site. During the inspection, the pit <br />excavation was measured to be 25 feet deep, approximately 9 acres of current disturbance <br />was observed and no asphalt or concrete plant was located at the site. The Ula Pit is in <br />compliance with the permit conditions. <br />The site was snow covered and no mining operations were occurring during the inspection. <br />The pit has been mined to the northern extent of the original 110 permit area. The north <br />slope was mined to a 3:1 slope and final reclamation is scheduled for this spring. <br />Reclamation will include topsoil replacement and seeding per the approved reclamation <br />plan. Mr. Beach estimated mining would remain in the 110 area for another year proceeding <br />to the east boundary. The required 25 foot property line offsets were verified along the <br />north and east sides of the site. Vegetated topsoil stockpiles were located in the <br />northeast corner and along the southern boundary of the original 110 permit area. No <br />evidence of erosion or exposed groundwater were observed during the inspection. <br />No fuel was stored on site. The only equipment observed on site was a front-end loader. <br />Mr. Beach stated the pit is contract mined with a mobile processing plant. <br />The Division currently holds a financial warranty of $30,995, which is adequate to <br />complete the reclamation of the site based on the observations from this inspection. <br />An entrance sign was posted and permit boundary markers were observed as required by Rule <br />3.1.12.
