<br />Document Imaging CD/DVD <br />REQUEST FORM & INSTRUCTIONS <br />Request for the DMG d c ent images on the following DMG permit file(s): <br />1) -I 002 2) - 3) <br />RECEIVED <br />--?FEB 05 2009 <br />Division of Reclamation, <br />Mining and Safety <br />4) <br />The DMG document images can be copied to either a CD or a DVD. Please check one below and return this <br />form with your payment to DMG. If a CD is requested and the permit is too large for a CD, you will be called. <br />Please allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. <br />CD @ $5.00 per disk DVD @ $20.00 per disk <br />Please check if you want a copy of the QuickView installation instructions. <br />Computer requirements to use the DMG document imaging CD (or DVD): <br />1. A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the <br />first time. Future use of this or other imaging CD's does not require a new installation of the software. <br />2. The CD or DVD that you purchase must be in the D: drive. A DVD drive will read a CD or DVD. <br />3. The CD will contain files with the tif extension. There needs to be an image viewing application for tif files on your computer that <br />will handle single and multi-page documents. On a Windows XP computer, the default viewing programs are called "Windows Picture <br />and Fax Viewer" or "Paint" and they do not work with QuickView. The Imaging Viewer that comes with pre-Windows XP computers <br />does.work. DMG has determined that a program called "IrfanView", which can be downloaded from IrfanView.com, does work with <br />QuickView. After downloading "IrfanView", it needs,to be configured as your default.tif viewer. DMG does not support or endorse this <br />software and the licensing and registration are the user's responsibility. There are many other programs that work as well, such as <br />"Imaging Preview" and "Imaging for Windows". <br />Instructions: <br />1. Insert the CD into your computer. <br />2. Allow time for the CD to install. <br />3. Eject the CD from the computer. <br />4. Reinsert the CD into the computer. <br />5. You are ready to view the Permit Files <br />NOTE: A program called QuickView Autorun, which runs the QuickView <br />setup program, has also been copied to this CD. If the Autorun program <br />does NOT work, you will need to manually install the QuickView program. <br />Instructions for manually installing QuickView are available upon request <br />with the CD or can be viewed at www.mininv.state.co.us. <br />The Permit File document images can be viewed in the reception area of the DMG office or they can be purchased on a CD or <br />DVD. The CD/DVD purchase cost covers the time and labor of making the CD.
