If there is no corporate seal, than <br />either write "no seal" or draw a seal. In addition, the signatures on this page must be <br />in blue ink. <br />6.4.1 Exhibit A - Legal Description and Location Map <br />No comment. <br />6.4.2 Exhibit B - Index Map <br />No comment. <br />6.4.3 Exhibit C - Pre-mining and Mining Plan Map(s) of Affected Lands <br />3. The Mining Plan Map does not include the type of present vegetation covering the <br />land. Please include this information as required by Rule 6.4.3(e). <br />4. Rule 6.4.3(g) requires the applicant to identify the owners of any significant, <br />valuable, and permanent man-made structures within two hundred feet of the affected <br />land. Please provide the owner's name with any associated structures on the Mining <br />Plan Map. <br />5. The Mining Plan Map does not show where the site access will be located. Please <br />include the site access on the Mining Plan Map. <br />6. The Mining Plan Map does not show the warm water canal that is located in the <br />southern portion of the permit area. Please include this canal on the Mining Plan Map. <br />7. The Mining Plan states that there will be designated areas for concrete truck washout <br />and excess concrete product dumping. Please show the location of these areas on the <br />Mining Plan Map. <br />6.4.4 Exhibit D - Mining Plan <br />8. The Mining Plan states that the slurry wall will be constructed prior to Phase 3 or <br />between Phases 3 and 11. Please provide a more specific timeframe as to when the <br />slurry wall will be constructed. <br />9. The Mining Plan states that final slope will be 3H:1 V down to a depth of 10 feet <br />below the normal water level. The applicant must demonstrate that the water level <br />will be maintained at a certain depth or range of depth. If the applicant is unable to <br />demonstrate that the water level will be constant, then the final slope will need to be <br />3 H:1 V for the entire length of the slope.
