(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-2009-006 <br />INSPECTION DATE: 02/03/09 INSPECTOR'S INITIALS: ECS <br />OBSERVATIONS <br />This was a routine pre operational inspection conducted by Eric Scott and Tony Waldron of DRMS for the Meadow Hollow Quarry <br />site. The DRMS representatives met with John Holliday and followed him to the site at approximately 1000. The site entrance is <br />located off of County Road 31 (Meadow Hollow Road) on the west side of the road approximately 3/4 mile after the pavement ends, <br />near a pre-law quarry site, and is accessed via a private gate. The site was marked and the required postings were present. <br />This site has been disturbed in several areas by pre-law sandstone quarry activity, and is located on an east facing slope. The site is <br />well vegetated with native grasses and pine trees in areas that have not been historically disturbed. DRMS representatives toured the <br />site with the site representative, discussed some of the minor adequacy issues with the application, and how they might be best <br />addressed. <br />It was noted that the majority of the site is steeply sloping to the east and runoff from a strong storm event may leave the site if some <br />runoff control measures are not implemented. Run-on control may help minimize potential site drainage issues as well, as there is <br />currently a natural runoff pathway through the site that could carry water during a major storm event.
