Outfall <br />005A/005X/MON005 flowed the third week of December. No permit exceedances were detected. Please note <br />that even though most sites did not discharge in December, one site (003A) did flow earlier during the <br />quarter and does require quarterly DMR reporting for a two parameters (conductivity & TDS). Thus, the <br />enclosed report for 003A contains this quarterly information, while the monthly reporting parameters <br />have been left blank since they did not flow in December. <br />A Quarterly Whole Effluent Toxicity (WET) test was collected for analysis for Outfall 005X during the fourth quarter. The test <br />indicated that the effluent were not acutely toxic to Daphnia magna survival (see attached laboratory report). <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the outfalls, which did not discharge in December 2008. This includes Outfalls 003A (Pond B), 004A (Pond C), and <br />007A/007X/MON 7 ( Pond F), and 008A (Pond G) <br />CERTIFICATION <br />I certify under penalty of law that / have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete. / am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment.
