Energy Fuels Resources Corporation <br />As Built Site Report <br />January, 2009 <br />1. Introduction <br />Energy Fuels Resources received a permit from the DRMS for the Whirlwind Mine in early 2008 <br />under permit number M-2007-044. Greg Lewicki and Associates was retained by Energy Fuels to <br />work with the site engineer and contractor to ensure that all site construction occurred according <br />to the approved plans in the permit. This work was started in the fall of 2008 and continued until <br />late November of 2008. I, Greg Lewicki, P.E., was integral in the design of the surface facilities <br />and was involved in the proposals given to bidders. I also actively worked with engineer Bruce <br />Norquist (of Energy Fuels) who worked on the site with the contractor during the entire <br />construction process. <br />Work was only active at the Whirlwind portal and facilities area. The Packrat portal area was <br />inactive and is not covered in this report. The ore pad and the water treatment facility is also not <br />subject of this report. <br />The disturbed area of the Whirlwind portal area, as of December, 2008, is 11.47 acres. All of the <br />.10 disturbed area is within the projected disturbed area of the approved permit. <br />2. Construction Items <br />The water treatment plant, mine bench, office trailer, shop, dry, security fences and other <br />associated facilities were constructed prior to the fall 2008 work. All of these facilities were <br />constructed according to the plans in the approved permit except that the security fence around the <br />water treatment area was extended slightly to the north. <br />During the fall 2008 site construction period, the following items were constructed: mine <br />sediment pond, diversion ditches, collection ditches, culverts associated with all ditches, topsoil <br />stockpiles from all site stripping, employee parking area, overburden stockpile (from pond <br />construction), and the mine septic system.
