(Page 2) <br />MINE ID # OR PROSPECTING ID #: tin-1987-038 <br />INSPECTION DATE: 1 gJ9in8 INSPECTOR, S INITIALS: -KAE <br />OBSERVATIONS <br />This inspection was conducted in response to a permit amendment application for an existing 112c <br />permit which proposes to add 15.6 acres to the existing permit area. Present at the inspection were <br />Kate Pickford, DRMS, Matt Carnahan. Consultant for the operator, and Peter Kearl, Oldcastle SW Group, <br />Inc. dba Four Corners Materials. Weather at the time of the inspection was clear, calm, and <br />approximately 35 degrees. <br />2. The proposed, affected areas to be added to the permit area are located along the west side, along <br />the northeast side and along the southeast side of the current permit area. The proposed areas along <br />the northeast southeast sides are currently pasture land. The area to the west consists of wooded land <br />adjacent to the west boundary of the property. <br />3. Groundwater is expected to be encountered during the mining of the west portion of the amended <br />area, which will result in the necessity of dewatering the pit. Cottonwoods are located on the west side <br />of the property line and there is likelihood that these trees would be impacted by dewatering of the pit <br />in this area. There are already monitoring wells installed in this area. Parameters for a groundwater <br />monitoring plan were discussed, including potential mitigation measures to be taken to prevent impacts <br />to the trees. These issues will be further addressed during the adequacy review of the proposed <br />amendment application. <br />4.
