ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-I9Rn-gad <br />INSPECTION DATE: 1/949 INSPECTORS INITIALS: RK, <br />OBSERVATIONS <br />The Division conducted a BI-MONTHLY monitoring inspection of the site on 1/2/09. Main <br />purpose of the inspection was to verify pond levels. <br />Present during the inspection was Mr. Mark Vanoni of CC&V. <br />Inspected Phases I,II,III&IV Leach pad areas and recorded the following readings from the <br />high and low volume solution collection transducers for the pregnant ponds. <br />Phase I High Volume solution collection transducer # 1 was recorded at 40.3 # 2 was at <br />49.2 and # 3 was at 49.6. Low volume # 1 was recorded at .35 and # 2 was at .37. <br />Piezometer for the pond was recorded at 56.1 feet. <br />Phase IT & III High Volume Solution collection transducer # 4 was recorded at 37.5 and # 5 <br />was at 31.6 and # 6 was at 37.1. Low volume # 1 was at .12 and # 2 was at .30. Piezometer <br />for the pond was measured at 38.8 feet. <br />Phase IV High Volume solution collection transducer # 7 was recorded at 41.4 # 8 was at <br />40.4 and # 9 was at 42.2 and # 10 was at 41.2. Low volume transducer # 1 was at 16.1 and # <br />2 was at 12.7 inches of hydrostatic head. <br />Inspected the External pond, it was holding about the same amount of partially frozen <br />water. The following readings were recorded from the low volume solution collection <br />transducers. Low volume collection transducer # 1 was recorded at 11.3 and # 2 was at <br />13.0. <br />Inspected the Phase IVC leach pad drainage enhanced area. It was covered with ice and snow <br />no problem noted. <br />Inspected the Cresson Mine area, both the main Cresson 9 south side )and East Cresson <br />Wild Horse area were active at the time of the inspection. The East Cresson Backfill was <br />progressing, and should be completed in the next few months. <br />Haul road storm water drainage structures for the area were found to be in good repair. <br />Inspected the old Iron Clad Globe Hill area pads. South and West side of these pads there <br />are a lot of mine related possibly salvageable waste scattered through out the area, it <br />will be a good idea to place all these mining related waste in one concentrated area. <br />Granted most of the area will be mined remind or mined out as part of the North Cresson <br />mine expansion, however, until such time and for the future it will be a good idea for <br />CC&V to concentrate all these mine related waste in one ideal location. <br />Looked at the only are of the Iron Clad Globe Hill pit that was not backfilled, these area <br />is expected to be mined as part of the North Cresson mine expansion, and will be reclaimed <br />as approved in Amendment#9 <br />Inspected the under drain discharge areas. All the ponds were as described in the last <br />inspection. Both the South under drain ((2gpm) and the 4 inch pipe at .45gpm) were <br />discharging. Looked at AG 1.5 it was dry.(Note: All piezometer readings unless noted are <br />in feet of hydrostatic head. <br />I & E Contact Address <br />NAME: MR.
