The name and permit number of the mine and the name and address of the permittee <br />are: <br />Carbon Junction Mine (CDRMS permit number C-92-080) <br />Oakridge Energy, Inc. <br />4613 Jacksboro Highway <br />Wichita Falls, Texas 76301 telephone (940) 322-4772 <br />b). No additional acreage was disturbed in 2008. None of the reclamation activities <br />undertaken in 2008 entailed additional disturbance. <br />c). Backfilling and final grading were completed in 2006. Backfilling and final grading <br />of the sediment control system (Pond 001, Pond 002, and ditches) was completed in <br />December 2008. <br />d). Topsoil replacement was completed in 2006. Topsoil replacement on the final <br />regraded areas of the reclaimed sediment control system was completed in December <br />2008. <br />e). Revegetation was undertaken during 2006 with successful establishment of the <br />perennial seed mix confirmed this year by investigations conducted by Michael Savage of <br />Savage and Savage, Inc.
