ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-91n <br />INSPECTION DATE: 91xi.Ing INSPECTORS INITIALS: -BK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2008-2009 calendar years. <br />1) Site is a large 110 acre 112 lime stone quarry located approximately 1 mile North of <br />Manitu Springs. Present during the inspection were Mac Schafer and Chris Usery the <br />operator's and mark Heifner there consultant. <br />2) Total permit area is 110 acres with total maximum affected area to be 85 acres. With <br />the exception of backfilling to 7200 feet elevation, no mining was taking place at the <br />time of the inspection. According to Mark Heifner, the operator is under negotiations <br />about the widening the access road. <br />3) Site has been active since per- ;aw and areas affected during the per- law years have <br />been incorporated into the permit. That includes a small water storage pond located along <br />the North side of the permit area. It was a lined pond, but most of the liner is no longer <br />in place. According to Mark, the operator will reclaim the area once the mining plan is <br />completed. <br />4) As stated the only activity at the time of the inspection was backfilling the active <br />mining area to 7200 feet elevation as amended in the 1994 reclamation plan. Operator <br />stated that once the access road issue is resolved and mining commences to the North West, <br />they will determine if they will repair the existing crusher and screen ( plant) that was <br />still in place at the time of the inspection, and move it to a northerly area to <br />accommodate the mining activity or place another crusher and remove the old one from the <br />permit area. <br />5) Drove the access road to the area along the North West side where mining will commence <br />once the road access issue is resolved. Mr. Heffner briefly explained the planned mining <br />sequence in the area. There was an old powder magazine (empty) noted along the north side <br />of the access road. North of the access road and east of the next phased mining area, is <br />where the old lined water storage pond used to be. As mentioned above most of the liner is <br />has degraded and the operator plans to reclaim the pond as part of their reclamation plan <br />for the site. Surface water drainage control for the area was found to be in good repair. <br />No major off site erosion features were noted at the time of the inspection. <br />6) When the site was last inspected in 2001, the only problem sited was compliance with <br />Rule 6.5(4).On July 24, 2000, the Division approved the TR incorporating the blasting plan <br />into the permit. The operator was supposed to provide their blasting company's <br />recommendation (Geotek"s) to monitor each blast with limits on PPV and maximum Db AIR <br />PRESSURE. The Division could not find the required submittal. If the operator had not <br />blasted since the TR was approved, then please submit a statement as such. However if <br />blasting had occurred since the approval of the TR, the DBMS would like all the records to <br />verify that the set standards were not exceeded. <br />7) According to the annual report and what was observed on site the total amount of <br />affected land is 74.8 of which 8.9 acres have been reclaimed. The total warranty for the <br />affected land that is currently in place is $340,000. Since the last time the warranty was <br />calculated was in 1994, DRMS request the operator to provide a revised warranty <br />calculation covering all the areas that will need to be covered. Please note the total <br />permitted affected area for the site can not exceed 85 acres. The revised warranty <br />calculation must also include the backfilling of the old water storage area, and all <br />access roads that will not be left after the mining is completed. The warranty calculation <br />must also include the demolition or removal of the shop building and all man made <br />structures such as the plant site.
