ORIGINAL - PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-i9Rn-9Qa <br />INSPECTION DATE: t <br />INSPECTOR?S INITIALS: -BK- <br />OBSERVATIONS <br />The Division conducted a bi monthly winter season monitoring inspection of the site on <br />12/01/08. There was measurable snow cover in the entire permit area. Present during the <br />inspection was Mr. Mark Vanoni of CC&V. <br />Inspection began at the phase IV low volume, solution collection pump area, where CC&V <br />placed a new frequency cycling low volume pump. The installation of this new pump is <br />supposed to eliminate or at least prevent from the head ever exceeding 2 feet. During the <br />replacement of the pumps, the low volume log that is kept in shed indicated the 2 feet of <br />maximum allowable head was exceeded as expected. <br />Inspected the recently enhanced solution collection perimeter ditch along Phase IV C pad, <br />it was snow and ice covered. Ditch appeared to be functioning as designed. <br />Inspected the phase I,II,III&IV LECAH pad areas and recorded the following readings from <br />the high and low volume solution collection transducers. <br />Phase I high Volume solution collection transducer # 1 was recorded at 49.6, # 2 was at <br />54.7 and # 3 was at 54.5. Piezometer for the pond was at 58.9 feet. Low volume transducer <br /># 1 was recorded at.33 and # 2 was at.35. The High volume readings are about 3 feet below <br />the 80% maximum allowable operational level. CC&V was notified to monitor the level during <br />the holiday week. ON 12, 24/08 Gary Horton informed DRMS over the phone that the Piezomter <br />reading for the and was at 59.3 feet of head. <br />Phase II High Volume solution collection transducer #4 was recorded at 39.2 # 5 was at <br />32.7 and # 6 was at 38.7. Low volume solution collection transducer # 1 was recorded at.15 <br />and # 2 was at.47. Piezomter for the pond was at 41.5 feet. <br />Phase IV high volume solution collection transducer # 7 was recorded at 45.8,4 8 was at <br />46.0 and # 9 was at 48.7 and # 10 was at 46.3 . Low volume transducer # 1 was recorded <br />at19.0 and # 2 was at 15.4 inches of hydrostatic head. <br />Inspected the external pond, it was holding what appeared to be a partially frozen water, <br />with snow cover, was noted at the time of the inspection. Low volume solution collection <br />transducers for the pond were measured at 15.9 and # 2 was at 15.1. <br />Inspected the under drain area, both the south and 4 inch pipes were discharging. All <br />lower ponds were holding some snow and a small amount of frozen water in addition to show <br />at the lowest pond. <br />Inspected AG1.5, IT WAS DRY. <br />I & E Contact Address <br />NAME: MR.
