5 (Fonn 2- .Public)
G. Total project area to be disturbed < 5 acres (acres)
----------- (acres)
11. Describe the equipment to be used for the prospecting operations:
Intermittent and temporary surface equipment may include a dozer; an excavator, and a loader to open adits, construct berms,
and maintain waste stockpile slopes at <3:1. Also an electric generator and compressor. Underground equipment may include
a bobcat and ore cart.
1. Describe and locate any structures to be constructed (i.e. stockpiles, ponds, impoundments):
Waste and ore stockpiles as shown on maps in Exhibit will be established in existing disturbed area with berms constructed
to prevent run-on and run-off.
J. Describe anticipated relationship to surface water and groundwater (proximity to streams, penetration of ground
water aquifers):
No ground or surface water is anticipated. Historical and current mining in the area is dry and Nuvemco drilled two
............. --
groundwater monitoring wells which were dry. These wells will be monitored monthly for an adjacent permit. If exploration
------ - - - ------ - ----- ------ - ----
drilling encounters groundwater, it will be sampled and an additional monitoring well will be constructed.
1V. OPERATION AND RECLAMATION MEASURES:
1. The Board suggests that a photographic record of the pre-prospecting and post-prospecting conditions be kept by the
prospector. "These photos should be taken from the same location and by the same method to clearly show the pre-prospecting
condition of the land and the reclamation efforts. Upon completion of reclamation and request for bond or surety release, the
Board may consider the photos as evidence of adequate reclamation, and thus, be able to act more quickly on the request for
release. In addition to the aerial photographic basemap, numerous photos have been taken, some of which are attached in Exhibit C.
Additional photographs will be taken prior to drilling
2. Provide a description of the native vegetation of the area to be disturbed including tree, shrub, and grass communities of the
area. Color photographs, sufficient to adequately represent the ecology of the site and adequately labeled (including date,
orientation and location), may be used in lieu of a written. description. Based on the quality of the photographs, the Division
may require additional detail.
Old reclaimed mine site with limited to no vegetation (see photographs in Exhibit C).
-----------.._._,...- ........ _..... -._._............ ._..-........... ._...... °_ - •.._.-..__.. -....,.......__..._..----------
Reclamation will include scarifying the ground prior to broadcasting the recommended seed mixture (Exhibit D). The portal
___._.__.......__._..._._._..._----- ._
area will be gated and locked.
