STATE OF COLORADO
DIVISION OF RECLAMATION, MINING AND SAFETY
Department of Natural Resources
1313 Sherman St., Room 215
Denver, Colorado 80203
Phone: (303) 866-3567
FAX: (303) 832-8106
April 18, 2008 `?
C)9'
COLORADO
D IV IS I ON OF
RECLAMATION
MINING
SAFETY
Ms. Deanna Van Ella
180 Lane 200 Bass Lake
Angola, IN 46703
RE: Possible Succession of Operator, Gold Basin Mine, Gold Mountain Metals, Inc.
as operator, Permit M-1992-045.
Dear Ms. Van Ella:
Bill Ritter, Jr.
Governor
Harris D. Sherman
Executive Director
Ronald W. Cattany
Division Director
Natural Resource Trustee
I have enclosed a Succession of Operators Application Package for your use along with a number of
different kinds of Reclamation Bond forms for your use. These forms are also available over the internet at
htW://rnining.state.co.us. The site remains under the same permit as when you initially operated the
mine.
At this time I do not have a specialist managing the Gold Basin site. The last inspection was conducted on
August 25, 2006 in response to a complaint that the Division of Reclamation, Mining and Safety received
on August 10, 2006. The inspection resulted in the Division requiring Gold Mountain Metals, Inc. to file a
technical revision (TR-02) that requires the operator to run a closed circuit water recirculation system to
operate the sluice box at the mine. I have enclosed copies of all documents regarding this complaint,
inspection and revision for your files.
Please be aware that any changes to a permit, including a succession of operator, must be initiated and/or
approved by the current permit holder.
Sincerely,
Carl B.
