Enclosed are the following <br />items as requested by Division letter of September 10, 2008: <br />1. The permit renewal application form with a statement of the address of the <br />permittee, the term of the requested renewal, and the permit number. <br />Additional enclosures include Map 3 Permit Boundary 2008 and a revised <br />Table of Contents - Page vii Maps. <br />2. A copy of the proposed public notice for Division review. Publication in <br />the Trinidad Chronicle News will commence upon notification from the <br />Division. <br />3. A copy of the current liability insurance certificate with an expiration date <br />of 2/08/2009.
