Rule 6.4.19 Exhibit S - Permanent Man-Made Structures <br />Question 12 Please Inform the Division if any roads will be mined through or relocated as a result of the <br />AM-01. Please state the owner(s) of the road(s) and provide a notarized consent form as required by <br />Rule 6.4.19. <br />12. The only road that might be affected by the mining operation in the future is Lake County Road <br />11A. Lake County Road 11A dead ends at the present mine gate. Therefore, mining through this road <br />will not affect any man made structure that the County does not own.
