A copy of "the .complete, revised Leyman Henn report is attached as <br />Exhibit 111. 14. A memorandum summarizing results. of slope: stability evaluations and a <br />copy of slope stability calculations are included in Exhibit IVA I -for DRMS review. <br />15) The breaks in the highwalls will be between 50 and 75• feet long. The previous. <br />Trench and. HighwaIS chematic (Exhibit D-1) has been revised and is included in Exhibit. <br />111. 15. Note that these breaks will be at random locations. <br />16) The operator understands that any changes to the high wall and bench configuration, <br />other than 'what is submitted with this-amendment application, must be submitted in the <br />form of a Technical Revision to the permit. <br />IV) EXHIBIT E Reclamation Plan <br />1) Applicable portions of the "Rules and. Regulations for Dam Safety and Dam <br />Construction," "State Engineer Guidelines for Lining Criteria for Gravel Pits," and <br />"Guide to Specification Preparation for Slurry Walls and Clay Liners as-a Component of <br />a Colorado Mined Land Reclamation Permit" are included in Exhibit IV. 1. Please refer <br />to the QA/QC material for design and test specifications guidelines. The clay blanket <br />will be approximately 15-feet thick and will be placed along the 0.5:1 mining highwall. <br />Overburden will be placed on this liner to create the-3:1 slopes. A copy of the- cross <br />section design is included in Exhibit IV. 1. The approval letter from the Office of the State <br />Engineer will be provided to the DRMS upon receipt. <br />2) In the event the Office of the State Engineer's. approval is- not granted, an at-grade <br />reservoir- storing approximately 2,500 acre-feet will be :constructed using the same 15- <br />foot clay blanket liner design. A Technical Revision will be submitted at-a later date if' <br />the reservoir design is modified. No jurisdictional structure will be constructed prior to <br />approval. <br />3) . The 2007 JA Cesare Report provides data that supports the conclusion that the <br />western bedrock material will significantly seal the reservoir to meet the State Engineer's <br />requirement. A copy of the report has previously been provided to the DRMS. <br />4) A copy of the Office-of the State Engineer approval letter and the QA/QC procedure <br />for the installation of the clay liner will be provided to .the DRMS upon SEO- approval.. - <br />General QA/QC information regarding clay liners has been provided, <br />5) A description of reclamation phases for use in the bond calculation is attached in <br />Exhibit VI. <br />6). The Office of the State Engineer "Dam Safety Design Criteria" will provide guidance <br />for backfill material compaction as well as. testing frequency to achieve 95% proctor <br />compaction. The applicable portions of the "Rules and Regulations for Dam Safety and <br />Dam Construction" are included in Exhibit IV.1 <br />7) In lieu of permanent. inlet and outlet structures, the operator intends to utilize portable <br />pumps as necessary until. an end user is identified.
