MINE ID # OR PROSPECTING ID #: M-1923-041 <br />INSPECTION DATE: 11-1?-9MA <br />INSPECTOR=S INITIALS: 11F <br />BMK <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when <br />considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />Problem/Possible Violation: February <br />1') The current mine plan needs to be updated and clarified. Pursuant to C.R.S. 34-32.5-112 18th, 2009 <br />(1) (c) (VI). The operator must provide sufficient information to describe or identify how <br />the operator intends to conduct the operation. <br />Type PB <br />Topic I MP <br />Corrective Action: <br />Please submit a Technical Revision to update and clarify the current approved mine <br />plan. The Technical Revision must include the information that is described in the <br />observations portion of the inspection report and is summarized on Page 3 of the report. <br />Problem/Possible Violation: February <br />2') Water has been retained in the Ridge 4 excavation. This is a problem for failure to 18th. 2009 <br />prevent disturbances to the prevailing hydrologic balance of the affected land and the <br />surrounding area and to the quantity and quality of water in the surface and ground <br />water systems both during and after the mining operation and during reclamation <br />pursuant to C.R.S. 34-32.5-116 (4)(h). Specifically for, not obtaining an approved well <br />permit or substitute water supply plan to account for evaporative loss for the water that <br />has been retained in the it for longer than 72 hours. <br />Type PB <br />Topic HB <br />Corrective Action: <br />Also, if the water is to be pumped out of the pit, please submit a copy of the permit <br />obtained from the CDPHE for the discharge of the water. In any case, please submit <br />evidence that a well permit and substitute water supply plan has been applied for and/ <br />or approved by the Office of the State Engineer.
