PERMIT A <br />PERMIT TO PLUG & ABANDON MONITORING WELL(S) <br />WELL IDENTIFICATION: TR-4 <br />DATE: November 26, 2008 PROJECT: 1999-144 <br />PROJECT DESIGNATION: Chevron Mining Inc., Edna Mine COUNTY: Routt <br />I (we), the undersigned landowner(s) or agent grant to WWC Engineering the right to enter upon my <br />property for the purpose of plugging and abandoning monitoring wells in conjunction with <br />completing the above listed project. This entry pertains to the use of men, equipment, and passenger <br />vehicles and trucks of WWC Engineering and/or Rogue Resources Inc. Operations are to include <br />taking of photographs, survey of well location, removal of well casing and/or surface completion, <br />plugging of monitoring well with approved grout/sand/fill, and marking the location of the former <br />monitoring well with a stamped aluminum cap on rebar. <br />This permission does not constitute a waiver of any right or claim I might have against WWC <br />Engineering in the event of damage inflicted to my property as a result of WWC Engineer's entry. <br />Entry will be made during reasonable hours and after prior actual notice to the landowner of times of <br />entry. Entry will be accomplished peaceably and without inflicting substantial injury. <br />This permission may be revoked by me with written notification to: Edna Mine Project Manager, <br />WWC Engineering, 611 Skyline Road, Laramie, Wyoming 82070.
